Quick tips about claims and prior approval

To help us process your prior approval or claim as quickly as possible, please:

  • Include a copy of your GP referral letter.
  • Make sure your GP referral indicates the dates you first started having symptoms and why you first sought treatment for the condition you are claiming for.
  • Make sure to get an estimate of treatment costs from your specialist if you are applying for prior approval for a procedure.
  • Complete the form to the best of your ability and remember to provide us with the most up-to-date information.
