Please let your Adviser or Sovereign know as soon as you become aware of a potential claim.
For life insurance claims we require:
- An original or certified copy of the death certificate
- Proof of age (such as a certified birth certificate or passport). We may require additional information, depending on the circumstances of the claim.
For disability, critical illness, income protection and total permanent disablement claims we require:
- A signed and completed claim form
- We may require additional information, depending on the circumstances of the claim.

Once we have received all the necessary information to make a decision, it may take up to five working days before we are able to let you know our decision. For complicated claims it may take longer, in which case we will keep you informed of our progress.
