This form should be used to complete your application for life insurance or health insurance with us.

General Forms
Authority to Accept Direct Debit
Be sure to list all policy numbers that you want this direct debit to apply to. Then post the completed form to us and we'll do the rest. Direct debit is Sovereign's recommended payment method. Direct debit forms must be posted to Sovereign as the banks require us to hold the original copy. We need to receive your direct debit form at least ten working days prior to your next premium due date. Direct Debit Authority Form (PDF)
Authority to Operate
This form enables policy owners to authorise a stated individual limited authority as documented on the form for a specified duration of time. Authority to Operate (PDF)
Credit Card / Debit Card Payment Authority
Sovereign can accept Visa, MasterCard or debit card premium payments for some kinds of policies. To find out if you are able to pay your policy by credit card or debit card, please contact us. If you currently pay your premiums by credit card or debit card and wish to update your card details please complete this form. Declaration of Continued Good Health
When a Declaration of Continued Good Health has been requested by Sovereign, we require a completed form for each life assured on the policy. Completed forms can be posted to us or faxed to 0800 768 435 (+64 9 487 8033 from overseas). Declaration of Good Health (PDF)
Loss of Policy Declaration
If you have lost your policy documents and would like to get a replacement you can call us and we can organise it over the phone. Alternately, you can complete the Loss of Policy Declaration form and scan/email, fax or post it to us. We will send you a replacement document. Loss of Policy Declaration (PDF)
Transfer of Ownership
To change the ownership of a policy the current policy owner(s) and new policy owner(s) need to complete the form and post to us along with your policy documents. You can also find the form at the back of your policy documents. Transfer of Ownership (PDF)
Change of Name Declaration
If you have recently changed your name, you need to let us know. To change your name you will need to complete the form and send or fax us along with a copy of any of the following documents: Birth certificate, Marriage certificate, Annulment/ Divorce proceedings, Deed Poll, Passport endorsed with change of name, Adoption papers or Drivers licence. Change of Address
If you have recently moved, you can update your address details by contacting us by phone, or by completing the Change of Address form and sending it to us by post, email or fax. Change of Address (PDF)
Child Health application
To add a child aged 3 months or more to a Sovereign Health policy, complete the Child's Supplementary Application Form Child Health Application (PDF)
Special Events Application
If you’ve recently experienced a major life event, such as having a baby, getting married or buying a new home, you may want extra protection. Our Special Events Increase Facility and Special Events TPD/Trauma Facility allow you to increase your cover without having to provide further medical information, when a special event occurs. To increase your cover simply fill out this form and scan/email, fax or post to us, along with evidence of the special events shown on the application form. We will process your request and notify you of the premium increase for the extra cover. Special Events application (PDF)
Home Loan forms
Authority to Accept Direct Debits
To make payments to your Go Home Loan account from your account at another bank you will need to complete the Authority to Accept Direct Debits form and return to Sovereign Home Loans prior to your first instalment date. Authority to accept Direct Debits (PDF)
Authority to Operate
This form can be used by a Go Home Loan account holder to delegate authority or authorise another person (called the agent) to operate certain features of the account on the account holder's behalf. It has limited powers and is commonly used when an account holder wishes to allow someone else to operate their bank accounts in their absence.

Certificate of Non Revocation
A Certificate of Non Revocation is a declaration by the appointed attorney that as at the date of the declaration the Power of Attorney has not been revoked. International Money Transfer (IMT) Form

Lending Forms Library
Mortgage Memorandum - This form may be used for disclosure purposes relating to mortgage instruments which incorporate this memorandum number.

Land Deed -
These documents regulates priority between land mortgages. The forms can be downloaded by clicking on the form name.

Requirements for Identification

From Sovereign Superannuation transactions and Loan applications on some products you will be asked to verify your address and provide proof of identification.  Please read the Quick Reference Guide for further details on what to provide. This guide explains how to have a document certified and gives details for: 

(1) Individuals
(2) Companies
(3) Trusts
(4) Any person acting on behalf of another.

Once the Documents have been certified by a trusted referee, please ensure to post the physical copy of the original to your Adviser and/or Sovereign, to be held on file.
